Three of those you are not going to change. Your house is a moderate temperature — that is the point of a house. It has oxygen in it. And its food is anything organic, which is most of what it is built from: the paper on drywall, wood, dust, carpet, the backing on insulation. Breaking organic material down is what mold is for. That is its job in nature, and your house is made of the things it eats.
Which leaves water. Moisture is the only one of the four you control, and it is the only one that matters.
Dormant and active
Spores are already in your house. They are in every house, they came in through the doors and windows and on people, and that is normal and unavoidable.
What they do not do is grow, until they land somewhere with all four conditions met. A spore in a dry room stays dormant — it is still there, it has simply not switched on.
Give it water — a leak, or just air above about 60% relative humidity — and it switches on. It starts consuming the material it landed on, and it starts reproducing.
The smell is the announcement
When mold goes active it releases gases as it feeds. That is what a musty smell is: not the spores themselves, but the byproducts of mold that is eating something.
So the smell is not a general warning that mold exists somewhere. It is a specific signal that mold is active right now, in a material, nearby. Dormant spores have no smell. If you can smell it, it is growing.
How the copies move
Active mold makes copies of itself and releases them into the air. From there they go wherever the air goes.
People. Walking past a colony stirs the air enough to lift spores. Every door that opens moves air. So does every fan.
And the air conditioner, which does more than everything else combined. It is the largest air mover in the building by an enormous margin. It pulls air from the house through the return, across a coil that is wet by design, and pushes it back out through supply vents into every room. A system pulling air past active growth is a distribution network for it — and a system with growth inside the ductwork or on the coil is distributing from the source.
That is the single most important thing to understand about how a house gets contaminated. The mold does not creep along the wall from room to room. It gets picked up and delivered.
The CDC names the same route: mold "can enter your home through open doorways, windows, vents, and heating and air conditioning systems." What it does once it is inside a system that runs all day is the part homeowners rarely hear.
Cutting it out is the biggest release of all
The one thing that puts more spores into a house than anything else is somebody opening it up.
Mold sitting inside a wall cavity is contained by the wall. Disturbing it — cutting, scraping, tearing out material — releases an enormous quantity of spores into the room air all at once, and the air in that room goes everywhere the air conditioner takes it.
This is why containment exists. Seal the work area, put it under negative pressure so air flows into the area rather than out of it, and shut down the system so it is not moving air while the work happens. Done that way, what comes out of the wall stays in the work area.
Done without it — by a company skipping steps, or by a homeowner having a look — a problem in one wall becomes a problem in the whole house in an afternoon. We see the result of that more often than we see the original problem.
Why the room with the smell is often not the room with the problem
Once you understand the system as the distributor, this stops being mysterious.
A bedroom with no leak, no water damage and nothing wrong with it can smell musty because it is downstream of a supply vent, and the supply side is carrying what the return picked up somewhere else — or what is growing on the coil.
It is also why chasing the smell from room to room rarely works, and why a single air sample from one room tells you less than people think.

What actually stops it
Not killing spores — not because it cannot be done, but because it does not hold. Spores can be killed and they can be removed from a house. We do both. The house is not a sealed environment, though, so more come in through the doors, the windows and on people, the way they always have.
So killing what is in the air today buys you today. What decides whether they do anything once they land is the water. Every one of the four conditions is fixed except that one, so the whole job is finding the moisture, stopping it, drying the material, and removing what has already been consumed.
That is also why anything sprayed on the surface without fixing the water is a delay rather than a repair. The other three conditions have not changed, and the fourth is still there.

Can mold spread from room to room?
Yes, and the air handling system is usually how. Spores are moved by air, and the largest mover of air in a house is the air conditioner.
Does mold spread faster in some conditions?
It becomes active when moisture is present and stays dormant when it is not. Above roughly 60% relative humidity, growth becomes possible with no leak at all.
If I clean the visible mold, is it gone?
Not necessarily. You cleaned what you could see. Spores are left behind whether you can see them or not, and dormant ones simply sit there waiting for the conditions to come back. If the water is gone you may never see growth again, and that is a real result. But it is the water that produced it, not the wiping — and if the moisture is still there, so is everything else mold needs.
